The report is generated when the OK button is clicked on the Generate TIP File - Step 1 screen.

Data Elements

This section describes the data elements that are stored in the TIP file.

Agency Code

This value is the State Agency Code.

Origin of Data Element

The value is taken from the State Agency Code column of the TIP Report file.

Format

The value prints as a 2 character string, left justified.

Fiscal Year Code

This value is the fiscal year selected on the Generate The Integrity Profile (TIP) screen.

Origin of Data Element

The value is taken from the Fiscal Year column of the TIP Report file.

Format

The value prints as a 4 character string, left justified.

Vendor ID

This value is the ID of the vendor.

Origin of Data Element

The value is taken from the Vendor-ID column of the TIP Report file.

Format

The value prints as a 12 character string, left justified with trailing spaces.

New Vendor?

This value indicates that the vendor was initially enrolled in the WIC program during the selected fiscal year.

Origin of Data Element

The value is determined using the New-Vendor column of the TIP Report file.

Format

The value prints as an "X" for True and a space for False.

Training

This value indicates that the vendor attended either a new vendor training session or an annual training session during the selected fiscal year.

Origin of Data Element

The value is determined using the Training-Performed columns of the TIP Report file.

Format

The value prints as a 1 character string.

Type of Vendor

This value indicates the vendor's store type.

Origin of Data Element

The value is based on the Vendor-Store-Type column of the TIP Report file. The StoreType table VendorType is based on the TIP report file.

Format

The value prints as a 1 character string.

Risk Designation

This value indicates that the vendor was determined to be high risk during the selected fiscal year.

Origin of Data Element

The value is taken from the Risk-Designation column of the TIP Report file.

Format

The value prints as a 1 character string.

FNS Number

This value is the Food Stamp Authorization number of the vendor.

Origin of Data Element

The value is taken from the Food-Stamp-Authorization-Number column of the TIP Report file.

Format

The value prints as a 7 character string, left justified.

Vendor Name

This value is the name of the vendor.

Origin of Data Element

The value is taken from the Vendor-Name column of the TIP Report file.

Format

The value prints as a 50 character string, left justified with trailing spaces.

Street #

This value is the street number of the vendor's physical address.

Origin of Data Element

The value is taken from the Vendor-Street-Number column of the TIP Report file.

Format

The value prints as an 8 character string, left justified with trailing spaces.

Street Name

This value is the street name of the vendor's physical address.

Origin of Data Element

The value is the concatenation of the Vendor-Street-Name column of the TIP Report file.

Format

The value prints as a 40 character string, left justified with trailing spaces.

Address 2

This value is the vendor's additional address.

The value is the concatenation of the Vendor-Additional-Address column of the TIP Report file.

Format

The value prints as a 40 character string, left justified with trailing spaces.

City

This value is the city where the vendor is located.

Format

State

This value is the state where the vendor is located.

ZIP

This value is the ZIP code in which the vendor is located.

This value is the extended ZIP code in which the vendor is located.

Annual WIC Redemptions

This value is average amount of WIC benefit redemptions per month for the vendor during the selected fiscal year.

Format

The value prints as a 12 character string, left justified with trailing spaces.

Redemption Months

This value indicates the number of months the annual WIC redemptions cover.

Origin of Data Element

Format

The value prints as a 2 character string, left justified with leading zero.

Redemption Indicator A

This value indicates if the vendor had extremely small amount of variation in food instrument prices - the individual food instrument prices do not vary substantially from the average price.

Format

The value prints as a 1 character string.

B

This value indicates if the vendor had a large percent of food instruments redeemed at the same price.

C

This value indicates if the vendor had unusually high average food instrument prices - submitting extremely high average food instruments compared with similar vendors.

D

This value indicates if the vendor had redeemed prices higher than their price list.

Volume Indicator E

This value indicates if the vendor had large percent of high-priced food instruments.

This value indicates if the vendor was flagged for investigation because of volume of WIC business.

This value indicates if the vendor had large increase of dollar volume of food instruments redeemed over time.

Participant Indicator L

This value indicates if the vendor had Participant/other complaints.

This value indicates if the vendor had large number of participants redeeming food instruments outside of their health service area.

This value indicates if the vendor had large number of participants redeeming food instruments who are considered to be at high health risk.

This value indicates if the vendor had excessive number of returned checks due to errors.

This value indicates if the vendor had past history of violations and disqualifications.

This value indicates if the vendor had associations with known violators.

This value indicates if the vendor had multiple ownerships that include known violators.

This value indicates if the vendor was selected for investigations for other reasons.

# Of Routine Monitoring Visits

This value indicates the number of monitoring visit event or follow-up activity occurred.

Compliance Investigations

This value indicates that vendor received one or more compliance buys or follow up activity is recorded on the Vendor Folder.

Format

The value prints as a 1 character string.

Investigations by other Entities

This value indicates if the vendor had compliance investigations conducted by an outside agency, such as another State agency or the Food Stamp Program, or a Federal Law Enforcement agency.

Format

The value prints as a 1 character string.

# of Compliance Buys

This value indicates the total number of compliance buys the vendor received.

# of Inventory Audits

This value indicates the total number of vendor audit events or follow-up activities.

Reason for Sanction T

This value indicates that the vendor was found to be trafficking food instruments.

A

This value indicates that the vendor was found providing alcohol or alcoholic beverages or tobacco products in exchange for food instruments.

I

This value indicates that the vendor claimed reimbursement for the sale of an amount of a specific supplemental food item that exceeds the store's documented inventory of that supplemental food item for a specific period of time.

O

This value indicates that a violation was recorded for the vendor, where the vendor intentionally or unintentionally charged more for authorized supplemental food items than is permitted under the vendor agreement on a compliance buy check.

R

This value indicates that the vendor received or redeemed food instruments outside of authorized channels, which may include an unauthorized vendor and or an unauthorized person.

F

This value indicates that the vendor charged WIC for supplemental food not received by the participant.

U

This value indicates that the vendor provided unauthorized food items in exchange for food instruments, which includes charging for supplemental foods provided in excess of those on the food instrument.
