The Participant Summary (Output) allows the user to print a summary report of participant information for a selected certification period for the currently selected participant. The report is generated when the OK button is clicked on the Produce Participant Summary screen. The Start Date on the Produce Participant Summary screen is the minimum value of the certification effective date for the certification. The End Date on the Produce Participant Summary screen is the maximum value of the certification end date for the certification.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
SYSTEM ADMIN
The title of the report is the text "Participant Summary". A subtitle indicates the full name of the participant.
Origin of Data Element
The value is taken from the LastName, FirstName and MiddleInitial columns of the Member table.
Format
The value prints in {first name} {middle initial}. {last name} format.
The subtitle indicates the minimum value of the certification effective date and maximum value of the certification end date for the certification period for the participant.
Origin of Data Element
The certification period for the report is selected by the user on the Produce Participant Summary screen. These dates are the minimum certification effective date and the maximum certification end date for the certification period.
Format
The dates print in MM/DD/CCYY format.
Origin of Data Element
The value is the date on which the report is generated.
Format
The value is included in the following format: MM/DD/CCYY. The value will be the current system date when the report is generated.

The value is the time at which the report is generated.
Format
The value is included in the following format: HH:MM:SS (AM/PM).

This value is the State WIC ID of the participant.

The value is the date of birth declared by the participant during enrollment.

This value is the mailing address of the household of the participant.

The value is the MailAddress, MailCity, MailState, and MailZIP columns of the Household table.
Format
The value prints as its literal value.
This value is the telephone number of the household of the participant.
Origin of Data Element
The value is the Telephone1 column of the Household table.
Format
The value prints in "###-###-####" format.
This value is the full name of the authorized representative of the participant.
Origin of Data Element
The value is the AuthRepLastName, AuthRepFirstName and AuthRepMiddleInitial column of the Household table.
Format
The value prints in {first name} {middle initial}. {last name} format.
This value is the date from which the participant is certified and can utilize WIC benefits.
Origin of Data Element
The value is the CertEffectiveDate column of the CertContact table. It is the minimum value of the certification effective date for the certification.
Format
This is a date printed in MM/DD/CCYY format.
This value is the end date after which the participant is no longer certified and cannot utilize WIC benefits.
Origin of Data Element
The value is the CertEndDate column of the CertContact table. It is the maximum value of the certification end date for the certification.
Format
This is a date printed in MM/DD/CCYY format.
This value is the WIC Category of the participant for the most current certification record within the certification period.
Origin of Data Element
This value is the description associated with the CertWICStatus column of the CertContact table. (The most current certification within the certification period.)
Format
The value prints in its literal format.
This value is the service level that indicates the relative importance of serving the WIC participant.
Origin of Data Element
This value is the CertAssignedPriority column of the CertContat table. (The most current certification within the certification period.)
Format
This is a one-character value.
This value is the date that the height/weight measurements were taken.
Origin of Data Element
This value is the MeasurementDate column of the AnthropContact table.
Format
This is a date printed in MM/DD/CCYY format.
The value reflects the height of the participant in standard terms of feet and inches. Inches may include eighths of inches for infants.
Origin of Data Element
This value is taken from the LengthInches and LengthEighths columns of the AnthropContact table. If there are zero (0) eighths of an inch to report, the eighths measurement is not included.
Format
The value prints in a concatenated format of measurements and literals, for example 42 5/8 inches. If there are zero (0) eighths, then the height displays as "42 inches" for a measurement of 42 inches 0/8 eighths.
This value is the WHO (World Health Organization) Height for Age Percentile and reflects the percentile for the given height measurement and the age of the participant on the date that the height/weight measurements were taken. This value only prints if the WIC Category of the participant is either Infant or Child and the participant was less than 24 months old on the date of measurement.

The value is determined using the formula supplied by the Center for Disease Control and Prevention (CDC) and the following information:
DateOfBirth column of the Member table
Gender column of the Member table
CertWICStatus of the CertContact table
MeasurementDate, LengthInches and LengthEighths columns of the AnthropContact table
Format
The value prints in "###.#%" format if the WIC Category of the participant is either Infant or Child. Nothing prints if the WIC Category of the participant is any other value.
This value is the weight of the participant in pounds and ounces.
Origin of Data Element
The value is taken from the WeightPounds and WeightOunces column of the AnthropContact table.
Format
The value prints in a concatenated format of measurements and literals. For example, "147 pounds and 3 ounces" would be derived from 147 and 3 respectively in the WeightPounds and WeightOunces columns.
This value is the WHO (World Health Organization) Weight for Age Percentile and reflects the percentile for the given weight measurement and the age of the participant on the date that the height/weight measurements were taken. This value only prints if the WIC Category of the participant is either Infant or Child and the participant was less than 24 months old on the date of measurement.

This value is the body mass index for the height/weight measurements. It is only calculated if the participant was 2 years of age or older on the date that the height/weight measurements were taken.
Origin of Data Element
The value is calculated based on the following information:
DateOfBirth column of the Member table
Gender column of the Member table
MeasurementDate, LengthInches, LengthEighths, WeightPounds and WeightOunces columns of the AnthropContact table
Format
The value prints in "##.#" format if the participant is a woman or a Child who is 2 years old or older. Nothing prints if the participant is an Infant or a Child who is under 2 years old.
This value reflects the percentile for the BMI of the participant on the date that the height/weight measurements were taken. This value prints when the participant's age can be found on the CDC BMI percentile tables. This includes participants from the age of 2 years through the teenage years. The participant's age is based on the date the height/weight measurements were taken.

The value is determined using the formula supplied by the Center for Disease Control and Prevention (CDC) and the following information:
DateOfBirth column of the Member table
Gender column of the Member table
MeasurementDate, LengthInches, LengthEighths, WeightPounds and WeightOunces columns of the AnthropContact table
Format
The value prints in "###.#%" format if the participant is a Child who is 2 years old or older and for teenage women. Nothing prints if the participant is an Infant or a Child who is under 2 years old. For women who are not teenagers, the value "N/A" will print.
This value is the date that blood work was completed for the participant.

This value is the ResultDate column of the BloodWork table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the current hemoglobin value of the participant blood.
Origin of Data Element
This value is the Hemoglobin column of the BloodWork table.
Format
The value prints in "##.#" format. If the Hemoglobin value equals zero, blanks will be printed.
This value is the current hematocrit value of the participant blood.
Origin of Data Element
This value is the Hematocrit column of the BloodWork table.
Format
The value prints in "##.#" format. If the Hematocrit value equals zero, blanks will be printed.
This value is the current lead measure value of the participant blood.
Origin of Data Element
This value is the Lead column of the BloodWork table.
Format
The value prints in "###" format. If the Lead value equals zero, blanks will be printed.
The value reflects the current E.P. measure value of the participant blood.

This value is the EP column of the BloodWork table.
Format
The value prints in "###" format. If the EP value equals zero, blanks will be printed.
This value is the reason why the blood work measurements are not yet available for the participant.

This value is the description associated with the ExceptionReason column of the BloodWork table. The value comes from the Description column of the ReferenceDictionary table where the ExceptionReason equals the ExternalID and the Category = "BLDWRKEXCP".
Format
The value prints as its literal value.
This value is the certification effective date of the certification for the risk factors assigned during the certification period.
Origin of Data Element
This value is the CertEffectiveDate column of the CertContact table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the risk factor assigned to the participant. Risk factors may be system assigned or manually assigned.
Origin of Data Element
This value is the RiskFactorID column of the RISKFACTOR table for the associated CertContact table.
Format
This is a six-character value.
This value is the date that the food item was prescribed.
Origin of Data Element
The value is taken from the PrescriptionDate column of the FoodPrescription table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the quantity of the food item prescribed.
Origin of Data Element
The value is taken from the ItemQuantity column of the FoodPrescriptionItem table.
Format
The value prints in "##" format.
This value is the description of the food item prescribed to the participant.
Origin of Data Element
This value is the description associated with the FoodItemID column of the FoodPrescriptionItem table. The description comes from the Description column of the FoodItem table where the FoodItemID of the FoodPrescriptionItem table equals the FoodItemID of the FoodItem table.

The value prints as its literal value.
This value is the date when the education was conducted.
Origin of Data Element
This value is the ContactDate column of the EducationContact table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the topic of the education conducted.
Origin of Data Element
This value is the description associated with the TopicCd column of the EducationContactItem table. The description comes from the Description column of the EducationTopic table where the TopicCd, ContactMethod and ContactType columns of the EducationContactItem table equal the same columns on the EducationTopic table.
Format
The value prints as its literal value.
This value is the date when the education was conducted.

This value is the ContactDate column of the EducationContact table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the topic of the education conducted.

This value is the description associated with the TopicCd column of the EducationContactItem table. The description comes from the Description column of the EducationTopic table where the TopicCd, ContactMethod and ContactType columns of the EducationContactItem table equal the same columns on the EducationTopic table.

The value prints as its literal value.
This value is the first date that the set of food instrument can be used by the participant.
Origin of Data Element
This value is the FirstUseDate column of the FoodInstrumentSet table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the last date that the set of food instrument can be used by the participant.
Origin of Data Element
This value is the LastUseDate column of the FoodInstrumentSet table.
Format
This is a date printed in MM/DD/CCYY format.
This value is the number of instruments received by the participant in the set of food instruments.
Origin of Data Element
This value is an aggregate count of the number of records from the FoodInstrument table associated with the FoodInstrumentSet table for the FoodInstrumentSetID.
Format
This is a three-digit value.
Only information for the selected participant is included in the report.
The certification period may consist of multiple certification contacts. The following definitions are used when selecting data for this report.
Start date: This is the CertStartDate column of the CertContact table. In the case of pseudo certifications, all applicable CertContact records will have the same CertStartDate value.
Effective date: This is the CertEffectiveDate column of the CertContact table. In the case of pseudo certifications, each applicable CertContact record will have a different value in the CertEffectiveDate column.
End date: This is the CertEndDate column of the CertContact table. In the case of pseudo certifications, each applicable CertContact record will have a different value in the CertEndDate column.
All height/weight contacts where the MeasurementDate column of the AnthropContact table is between the certification start date and the maximum certification end date are included in the report.
All bloodwork contacts where the ResultDate column of the BloodWork table is between the certification start date and the maximum certification end date are included in the report.
All risk factors associated with all certifications for the selected certification period are included on the report. (The certification start date of the CertContact table equals the start date of the selected certification period.)
All food packages where the PrescriptionDate column of the FoodPrescription table is between the minimum certification effective date and the maximum certification end date are included in the report. If that selection does not include any food prescriptions, the food prescription with the most current prescription date less than the minimum certification effective date will be selected.
All education contacts where the ContactDate of the EducationContact table is less than or equal to the maximum certification end date and topics were assigned are included in the report. Contacts are grouped by Individual Contacts and Group Contacts.
Food instruments that meet the following selection criteria are included.
The FirstUseDate column of the FoodInstrumentSet table is between the minimum certification effective date and the maximum certification end date.
The VoidedDate column of the FoodInstrument table is null (not voided).
The ReportedStolenDate column of the FoodInstrument table is null (not reported stolen).
The data in this section will be sorted by Date.
The data in this section will be sorted by Date.
The data in this section will be sorted by Date then Risk Factor.
The data in this section will be sorted by Date then Description.
The data in this section will be sorted by Individual/Group, then Date then Topic.
The data in this section will be sorted by First Use Date then Last Use Date.
