The Enrollees by County/Municipality Report CAS001 (Output) consists of counts of enrollees in a valid certification at the time the report is requested. The report is generated when the OK button is clicked on the Generate Enrollees by County/Municipality Report screen.

The report will allow the user to print, by municipality, a summary report of the enrollment totals for the selected clinic(s) within their agency.

Data Elements

This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.

Agency

Origin of Data Element

The agency the user is signed on as when requesting this report determines this value. The agency code and description will be printed.

Format

This is a fifty-character text value. The format is determined by the information being reported.

County Name

Origin of Data Element

The selection criteria determine this value.

Format

This is a thirty-character text value.

State Cd

Origin of Data Element

The user records this value when the report is submitted.

Format

This is a two-character text value.

Municipality

Origin of Data Element

This value comes from the Name column of the LegalMunicipality table for each municipality with active enrollees at the time the report is submitted. The LegalMunicipalityID of the Household table is used to group the participants by municipality.

Format

This is a thirty-character text value.

Count

Origin of Data Element

This value is the count of enrollees, by municipality, that are in a valid certification at the time the report was requested.

Format

This is a six digit number.

Filter Criteria

Base of Data

All WIC enrollees eligible to receive benefits at the time the report is requested are included in the report totals. Enrollees must meet the following selection criteria.

Clinic(s)

The report will only include enrollees where the value of ServiceSiteID column of the Member table matches the clinic(s) selected by the user.

State

The report will only include enrollees where the value of StateCd column of the Household table matches the state selected by the user.

County

The report will only include enrollees where the value of CountyCd column of the Household table matches the county selected by the user. If all counties are selected, all counties within the selected state with active enrollees will be included in the report.

Sort Order

County

The report will be sorted by county name.

Municipality code

The report will be sorted by municipality name within county.

Control Breaks

County

On a change of county, a sum of the number of enrollees will be printed and a page break will occur.

Grand Total

No grand totals are defined for this system output.
