The Representative and Proxy History screen is used to view the previous authorized representatives and alternate representatives for the participant.

Unless otherwise stated below, all controls on the Representative and Proxy History screen are visible and disabled when:
The screen is active.
If one or more special conditions exist that affect a control on the Representative and Proxy History screen, the condition(s) are documented for that specific control below.
View the information in the Authorized/Alternate Representative Changes data grid.
The data grid displays all previous authorized representatives and alternate representatives or proxies for the participant. The entries are sorted in reverse chronological order according to the Change Date column.
The data grid consists of the following control(s):
Auth Rep Last Name column
This column displays the last name of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep Last Name". The information displayed within the column is read-only.
Auth Rep First Name column
This column displays the first name of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep First Name". The information displayed within the column is read-only.
Auth Rep MI column
This column displays the middle initial of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep MI". The information displayed within the column is read-only.
Auth Rep Relationship column
This column displays the relationship of the authorized representative to the participant for each record displayed within the data grid.
The title of the column is set to "Auth Rep Relationship". The information displayed within the column is read-only.
Auth Rep Marital Status column
This column displays the marital status of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep Marital Status". The information displayed within the column is read-only.
Auth Rep Education Level column
This column displays the maximum level of education of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep Education Level". The information displayed within the column is read-only.
Auth Rep Register to Vote column
This column displays the voter registration status of the authorized representative for each record displayed within the data grid.
The title of the column is set to "Auth Rep Register to Vote". The information displayed within the column is read-only.
Alt Rep 1 Last Name column
This column displays the last name of the first alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 1 Last Name". The information displayed within the column is read-only.
Alt Rep 1 First Name column
This column displays the first name of the first alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 1 First Name". The information displayed within the column is read-only.
Alt Rep 1 MI column
This column displays the middle initial of the first alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 1 MI". The information displayed within the column is read-only.
Alt Rep 1 Relationship column
This column displays the relationship of the first alternate representative to the participant for each record displayed within the data grid.
The title of the column is set to "Alt Rep 1 Relationship". The information displayed within the column is read-only.
Alt Rep 2 Last Name column
This column displays the last name of the second alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 2 Last Name". The information displayed within the column is read-only.
Alt Rep 2 First Name column
This column displays the first name of the second alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 2 First Name". The information displayed within the column is read-only.
Alt Rep 2 MI column
This column displays the middle initial of the second alternate representative for each record displayed within the data grid.
The title of the column is set to "Alt Rep 2 MI". The information displayed within the column is read-only.
Alt Rep 2 Relationship column
This column displays the relationship of the second alternate representative to the participant for each record displayed within the data grid.
The title of the column is set to "Alt Rep 2 Relationship". The information displayed within the column is read-only.
Changed By column
This column displays the name of the staff member who changed the record for each record displayed within the data grid.
The title of the column is set to "Changed By". The information displayed within the column is read-only.
Change Date column
This column displays the date the record was changed for each record displayed within the data grid.
The title of the column is set to "Change Date". The information displayed within the column is read-only.
It does not have a mnemonic. Its keyboard shortcut is the Esc (escape) key.
The Participant Folder screen displays when the button is clicked.
If data can be validated and saved on the screen, the following processes occur when the screen is processed:
A process to check for required controls as identified in the Data Map below is performed.
A process to check for valid entries as identified individually for each applicable control in Screen Elements above is performed.
A process to check for edits and cross edits as identified for each applicable control in Screen Elements above is performed.
If any checks or processes fail, a standard error message displays.
If no data can be validated and saved on the screen:
No cross edits are performed.
All values are considered legitimate.
No data is written to the database.
The Data Map defines the values saved for all controls on the screen. If available, any additional notes or comments are displayed in the Notes column.
