Monthly Termination Summary Report OPR017 (Output)

The Monthly Termination Summary Report OPR017 (Output) is a summary analysis of reasons enrollees were terminated from the WIC program. Participants are included in the report for the month that the termination occurred and are grouped by their category at the time of the termination. The totals do not include enrollees that are categorically ineligible. The report is generated when the OK button is clicked on the Generate Monthly Termination Report screen. This report will also be generated as part of the month-end process.

Month of

Origin of Data Element

The user records this value when the report is submitted. During the month-end process this is the CCYYMM value from the CurrentMonthEndDt column of the ProcessControl table.

Format

This is an accounting month and year formatted as Month Name CCYY.

Agency

Origin of Data Element

This value comes from the AgencyID and Name columns of the Agency table for the agency signed on as or selected.

Format

AgencyID is a three-character value and Name is a fifty-character value.

Clinic

Origin of Data Element

This value comes from the ServiceSiteID and ServiceSite Name columns of the ServiceSite table for the selected clinics.

Format

This value comes from the ServiceSiteID and ServiceSiteName columns of the ServiceSite table for the selected clinics.

Term Reason

Origin of Data Element

This is the reason for termination. A line is printed for every unique CertTermReason column of the CertContact table for selected participants. The reason description comes from the Description column of the Reference Dictionary table. The CertTermReason column of the CertContact table is used to get the Description column of the Reference Dictionary table where the category equals "TermReason" and the ExternalID equals the CertTermReason.

Format

The reason is a thirty-character value.

Pregnant

Origin of Data Element

This value is the count of selected participants where CertWICStatus column of the CertContact table equals "P".

Format

This value is a six-digit number.

Breastfeeding

Origin of Data Element

This value is the count of selected participants where CertWICStatus column of the CertContact table equals "B".

Format

This value is a six-digit number.

Non-Breastfeeding

Origin of Data Element

This value is the count of selected participants where CertWICStatus column of the CertContact table equals "N".

Format

This value is a six-digit number.

Infants

Origin of Data Element

This value is the count of selected participants where CertWICStatus column of the CertContact table equals "I".

Format

This value is a six-digit number.

Children

Origin of Data Element

This value is the count of selected participants where CertWICStatus column of the CertContact table equals "C".

Format

This value is a six-digit number.

Total

Origin of Data Element

This value is calculated by totaling the women, infants and children counts.

Format

This value is a six-digit number.

Filter Criteria

Base of Data

All participants terminated during the reported period that are not terminated for being categorically ineligible will be included in the report. Participants must meet the following selection criteria.

• The AgencyID column of the CertContact table equals the selected agency.

• The ServiceSiteID column of the CertContact table equals the selected clinic(s).

• The CertTermDate column of the CertContact table is within the reported month.

• The CertTermReason column of the CertContact table is not equal to "0".

Sort Order

Clinic ID

This report will be sorted first by clinic ID.

Termination Reason

This report will be sorted by the Term Reason (description) within clinic ID.

Totals for the clinic will be printed.
