Monthly Termination Detail Report OPR018 (Output)

The Monthly Termination Detail Report OPR018 (Output) is a detail analysis of reasons enrollees were terminated from the WIC program. Participants are included in the report for the month that the termination occurred and are grouped by their category at the time of the termination. The totals do not include enrollees that are categorically ineligible. The report is generated when the OK button is clicked on the Generate Monthly Termination Report screen. This report will also be generated as part of the month-end process.

Agency

Origin of Data Element

This value comes from the AgencyID and Name columns of the Agency table for the agency signed on as or selected.

Format

AgencyID is a three-character value and Name is a fifty-character value.

Clinic

Origin of Data Element

This value comes from the ServiceSiteID and ServiceSiteName columns of the ServiceSite table for the selected clinics.

Format

ServiceSiteID is a three-character value and ServiceSiteName is a fifty-character value.

Name

Origin of Data Element

The value will be taken from the LastName, FirstName and MiddleInitial columns of the Member table.

Format

The value will print in {last name}, {first name} {middle initial} format.

State WIC Id

Origin of Data Element

This value is the StateWICID column of the CertContact table.

Format

This is an eight-character value.

WIC Category

Origin of Data Element

This value is the CertWICStatus column of the CertContact table.

Format

This is a one-character value.

Term Date

Origin of Data Element

This value is the CertTermDate column of the CertContact table.

Format

This value will print in MM/DD/CCYY format.

Term Reason

Origin of Data Element

The value comes from the Description column of the Reference Dictionary table. The CertTermReason column of the CertContact table is used to get the Description column of the Reference Dictionary table where the category equals "TermReason" and the ExternalID equals the CertTermReason.

Format

The reason is a thirty-character value.

Filter Criteria

Base of Data

All participants terminated during the reported period that are not terminated for being categorically ineligible will be included in the report. Participants must meet the following selection criteria.

• The AgencyID column of the CertContact table equals the selected agency.

• The ServiceSiteID column of the CertContact table equals the selected clinic(s).

• The CertTermDate column of the CertContact table is within the reported month.

• The CertTermReason column of the CertContact table is not equal to "0".

Sort Order

Clinic ID

This report will be sorted by clinic ID.

Name

This report will be sorted by the Participant Name within Clinic ID (last name, first name, middle initial).

State WIC ID

This report will be sorted by the StateWIC ID within Participant Name.

Totals for the clinic will be printed.
