The Formula Usage Report OPR028 (Output) is generated to show how formula is being used throughout the state. The report is generated when the OK button is clicked on the Generate Formula Usage Report screen and the amount of information displayed on the report will depend on the options selected on that screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
Origin of Data Element
The food instrument type value is the code associated with the combination of distribution items and quantities on the food instrument. All food instrument types issued with formula for the reported month and with ProgramCode equal to the WIC value are listed. To determine if a food instrument was issued with formula, the DistributionItemID column of the FoodInstrumentTypeItems table is joined with the DistributionItemID column of the FoodDistCategory table. All food instruments in the reported month with a BaseFoodCategoryID value of "A" are included.

This is a five-character value.
Origin of Data Element
The value is the description of the formula item. The DistributionItemID column of the FoodInstrumentTypeItems table is used to get the Description column of the FoodDistributionItem table.

The value is the count of the total number of food instruments with formula for the FIType where the ParticipationDate column of the FoodInstrumentSet table is within the reported month and the ProgramCode column of the FoodInstrument equals the WIC value.

The value is the count of the total number of participants by FIType that were issued food instruments with formula where the ParticipationDate column of the FoodInstrumentSet table is within the report month and the ProgramCode column of the FoodInstrument equals the WIC value. participants are counted once per FIType.

The value is the count of the total number of food instruments by FIType with formula where the ParticipationDate column of the FoodInstrumentSet table is within the reported month, the PaidDate column of the FoodInstrument table is greater than zero, and the ProgramCode column of the FoodInstrument equals the WIC value.

The value is the count of the total number of participants by FIType that were issued food instruments with formula where the ParticipationDate column of the FoodInstrumentSet table is within the reported month, the PaidDate column of the FoodInstrument table is greater than zero, and the ProgramCode column of the FoodInstrument equals the WIC value. Participants are counted once per FIType.

The value is a six-digit number.
This is the average dollar amount of the FI Type that has been redeemed.
Origin of Data Element
The value is calculated by totaling the PaidAmt column of the FoodInstrument table for all food instruments for this FIType where the ParticipationDate column of the FoodInstrumentSet table is within the reported month, the PaidDate column of the FoodInstrument table is greater than zero, and the ProgramCode column of the FoodInstrument equals the WIC value. The total is divided by the Number Redeemed value for this FIType.
Format
The value is a five-digit number plus two decimal places.
Origin of Data Element
The value is calculated by dividing the Number Redeemed (line total) by the Number Issued (line total). The result is multiplied by 100.

The value is a three-digit number plus two decimal places.
This is the first name of the participant who was issued the FI Type.
Origin of Data Element
The value is the FirstName column of the Member table.
Format
This is a twenty-character value.
This is the middle initial of the participant who was issued the FI Type.
Origin of Data Element
The value is the MiddleInitial column of the Member table.
Format
This is a one-character value.
This is the last name of the participant who was issued the FI Type.
Origin of Data Element
The value is the LastName column of the Member table.
Format
This is a twenty-five-character value.
This is the address of the participant who was issued the food instrument with the reported FI Type.
Origin of Data Element
The value is the Address column of the Household table.
Format
This is a fifty-character value.
This is the city, state and zip code of the participant who was issued the food instrument with the reported FI Type.
Origin of Data Element
To get the city portion of the address, the LocalMunicipalityID column of the Household table is used to retrieve the Name column of the LocalMunicipality table. The state code will come from the Registry. These values are concatenated in city, state and zip code sequence.
Format
This is a fifty-character value.
The report is based on formula food instruments that were issued for the reported month. Formula food instruments have items with a base food category value of "A". Date for hidden agencies will not be included.
Agency, Clinic
If all agencies are selected, data for each individual agency will be included separately.
If an individual Agency and all Clinics are selected, individual Clinic data for that Agency will be listed separately.
If an individual Agency and individual Clinic are selected, the data for the selected Agency/Clinic will be included.
Category
If all category values are selected, data for each category will be listed separately.
If an individual category is selected, data for that category will be included.
Formula
If all formula items are selected, data for each formula item will be included.
If an individual formula item is selected, only data for that formula item will be included.
This report will be sorted by agency, Clinic, category, formula, and food instrument type, depending on selection criteria of the report.
If all agencies are selected, the report will be sorted by agency code.
If all clinics are selected, the report will be sorted by clinic code within agency.
If all categories are, the report will be sorted by category within clinic.
The report will be sorted by food instrument type, regardless of the other report selection criteria.
The detail report will be sorted by last name, first name, middle initial and State WICID.
On a change of Agency ide4ntifier a page break will occur.
On a change of WIC Category a page break will occur.
