The Formula Compliance Report NUT005 (Output) lists the number of infant and children participants redeeming food instruments containing formula issued for the reported closeout month. It compares the number of participants with contract, non-contract or special formula types with breakdowns by formulation (milk/soy and powder/concentrate/ready-to-use). The report is generated when the OK button is clicked on the Generate Formula Compliance Report screen. This report will also be generated as part of the month-end process.

The counts are accumulated by agency/clinic. Participants are counted in each agency/clinic that they were issued food instruments, if those food instruments were redeemed. Participants are grouped based on their category at the time the last food instrument was printed for the agency/clinic within the reported month. If a participant redeems more than one type of formula for the agency/clinic within the reported month they will be counted in only one category based on the formula issued on the latest printed food instrument for the agency/clinic/reported month.

The counts are accumulated in the End of Month process and are updated from the FoodItem table and updated into the ParticipantFormula table and used for this report.

Data Elements

This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.

Closeout Month of

Origin of Data Element

The user records this value when the report is submitted. During the month-end process this is the CCYYMM value from the CloseoutMonthEndDt column of the ProcessControl table.

Format

This is an accounting month and year formatted as Month Name CCYY.

Infants or Children

Origin of Data Element

When this report is requested, a report is produced for infant totals and a report is produced for children totals.

Format

This is a fifty-character text column. The format is determined by the information being reported.

Agency/Clinic

Origin of Data Element

The agency value comes from the AGENCYID column of the FoodInstrumentSet table. The agency name comes from the NAME column of the Agency table. The clinic value comes from the SERVICESITEID column of the FoodInstrumentSet table.

Format

The agency and clinic codes are three-character values. The agency description is a fifty-character value.

Contract Milk Concentrate

This column is calculated by totaling the number of distinct participants with redeemed food instruments issued for the reported month. The redeemed food instruments must have had milk based, concentrate, contract formula issued on it.

This column is calculated by dividing the total of the contract numbers (concentrate, powder and ready-to-use for milk and soy) by the total number of contract, non-contract, and special. The value is multiplied by 100 and rounded to the nearest tenth. The calculation is performed for every line item on the report.

Format

This is a three-digit number plus one decimal place.

Non-Contract Milk Concentrate

This column is calculated by totaling the number of distinct participants with redeemed food instruments issued for the reported month. The redeemed food instruments must have had milk based, concentrate, non-contract formula issued on it.

This column is calculated by dividing the total contract (number) by the grand total (number). The value is multiplied by 100 and rounded to the nearest tenth. This calculation is performed for every line item on the report.

Total Percentages - Total Non-Contract (number)

This column is calculated by totaling the number for non-contract milk concentrate, non-contract milk powder, non-contract milk ready-to-use, non-contract soy concentrate, non-contract soy powder and non-contract soy ready-to-use.

This is a six-digit number.

Total Percentages - Total Non-Contract (percent)

This column is calculated by dividing the total non-contract (number) by the grand total (number). The value is multiplied by 100 and rounded to the nearest tenth. This calculation is performed for every line item on the report.

Total Percentages - Special (number)

This column is calculated by totaling the number for special milk concentrate, special milk powder, special milk ready-to-use, special soy concentrate, special soy powder and special soy ready-to-use.

This column is calculated by dividing the special (number) by the grand total (number). The value is multiplied by 100 and rounded to the nearest tenth. This calculation is performed for every line item on the report.

Total Percentages - Grand Total (number)

Origin of Data Element

This column is calculated by totaling the number for contract and non-contract and special. This calculation is performed for every line item on the report.

Format

This is a six-digit number.

Filter Criteria

Base of Data

Infant and children participants that redeemed food instruments with formula items where the issuance month equals the closeout month are included in the report. Data for hidden agencies will not be included.

Sort Order

Agency

The report is sorted first by Agency.

Clinic ID

The report will be sorted by the Clinic ID within Agency ID.

Control Breaks

A page break will occur on change in category.

Grand Total

A total line for all infants and a total line for all children reported will be printed at the end of each section. The totals for infants and children will be totaled as grand totals on the last page of the report. Since participants can be counted in multiple agency/service sites, the grand totals reflect the sum of the rows on the reports, not the true count of distinct participants.

Frequency and Distribution

This system output is generated on demand. If selected by the state, this system output is also generated during the month-end process.
