The Contract/Non-contract Formula Report NUT003 (Output) lists the number of infant and children participants redeeming food instruments containing formula issued for the reported closeout month. It compares the number of participants with contract and non-contract formula providing breakdowns by formulation (powder/concentrate within milk/soy). The contract/non-contract ready-to-use formulation is combined and shown separately with breakdowns by milk/soy.
The counts are accumulated by clinic. Participants are grouped based on their category at the time the last food instrument was printed for the reported month. If a participant redeems more than one type of formula in the reported month they will be counted in only one category based on the formula issued on the latest printed food instrument for the reported month. The participant is counted only once.
Of the redeemed food instruments with formula for the reported month, if the last food instrument printed for the reported month is a contract formula, the participant is counted in the contract totals. If it is for non-contract formula, the participant is counted in the non-contract total. If it is for special formula, the participant is not counted.
The counts are accumulated in the End of Month process from the FoodInstrument, FoodInstrumentSet, and FoodInstrumentItem tables and updated into the ParticipantFormula table used for this report. The ParticipantFormula table is generated during the month-end processing.
The report is generated when the OK button is clicked on the Generate Contract/Non-contract Formula Report screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.

This is an accounting month and year formatted as Month Name CCYY.

Infant and children participants that redeemed food instruments with formula items where the issuance month equals the closeout month are included in the report. Participants are counted in the report if they meet the following selection criteria.
The ParticipationCYM column of the ParticipantFormula table equals the selected month.
The ProgramCode column of the ParticipantFormula table equals the WIC value.
The PaidDate column of the ParticipantFormula table is valued.
The WICStatus column of the ParticipantFormua table equals Infant or Child.
Participants are counted once per report.
If they are issued food instrument from multiple agencies/clinics, the participant is counted in the agency/clinic of the food instrument with the greatest value in the IssuedDate column of the ParticipantFormula table for the selected records.
If they are issued food instrument with multiple WIC status values, the participant is counted in the WIC status of the food instrument with the greatest value in the IssuedDate column of the ParticipantFormula table for the selected records.
Records for the ParticipantFormula table are generated during the month-end process.
This report will be sorted by WICStatus code. Children will print first, then infants.
This report will be sorted by agency code within WICStatus.
This report will be sorted by clinic code within agency code.
A page break will occur on change in category. A page break will also occur between the Contract/Non-contract/Ready-To-Use section and Total Percentages section.
A total line for all infants and a total line for all children reported will be printed. The totals for infants and children will be totaled as grand totals.
