Clinic Activity Report OPR016 (Output)

The Clinic Activity Report OPR016 (Output) is a summary analysis of the activities that occurred at each WIC clinic during a calendar month. It includes information regarding food instruments that were issued, voided and redeemed. The report is generated when the OK button is clicked on the Generate Clinic Activity Report screen.

Data Elements

This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.

Month of

Origin of Data Element

The user records this value when the report is submitted.

Format

This is a fiscal month and year formatted as {Month Name} CCYY.

Agency

Origin of Data Element

When reporting agency/clinic data, the Agency code and description will be printed. When reporting state data, the text description of "State Total" will be printed.

Format

This is a fifty-character text value. The format is determined by the information being reported.

Clinic

Origin of Data Element

When reporting agency/clinic data, the clinic code and description will be printed. When reporting state data, this value does not apply.

Food Benefit Pickup

Origin of Data Element

This value is the count participants with an issuance date in the reported month. This is the total count of unique State WICID's in the FoodInstrumentSet table where the IssuedDate column on the FoodInstrumentSet is equal to the reported month and the ProgramCode column on the FoodInstrument table equals the WIC value. The issued date is the date the checks were printed and not a participation period.

This value is a seven-digit number.

New Certifications

Origin of Data Element

This value is the count of enrolled participants new to WIC during the reported month. A participant is counted once in their lifetime. Participants must meet the following selection criteria to be included in this count:

ParticipationCYM column of the CaseloadCountEnrollment table equals the reported month
OrigEnrollmentDate column of the CaseloadCountEnrollment table equals the reported month

Subsequent Certifications

Origin of Data Element

This value is the total count of enrolled participants who were subsequently certified during the reported month. This count includes the Re-Certs-Prior Mo total. Participants must meet the following selection criteria to be included in this count:

Education Contacts

Origin of Data Element

This value is the count of participants who received nutrition education during the reported month. This value is the total count of unique State WICID's in the EducationContact table where the ContactDate column is equal to the reported month. The StateWICID and CertStartDate columns of the EducationContact table are linked to the StateWICID and CertStartDate columns of the CertContact table to select the agency and clinic where the participant received the education contact.

Services Provided

Origin of Data Element

This value is the total count of services provided by the clinic during the reported month. This value is calculated as the sum of the Food Benefit Pickup, New Certifications, Subsequent Certifications and Education Contacts for the reported month. Terminations are not included in this total.

Terminations

Origin of Data Element

This value is the count of participants who were terminated during the reported month. Participants must meet the following selection criteria to be included in this count:

Actual Participation

Origin of Data Element

This value is the unduplicated count of participants who participated during the reported month. Participants must meet the following selection criteria to be included in this count:

EnrolledThisMonth column of the CaseloadCountEnrollment table equals "Y"
ParticipationCYM column of the CaseloadCountEnrollment table equals the reported month
ProgramCode column of the CaseloadCount table equals the WIC value

Transfers In

Origin of Data Element

This value is the count of unique State WICID's who transferred into the clinic during the reported month. Participants must meet the following selection criteria to be included in this count:

EffectiveDate column of the TransferHistory table equals the reported month
ToAgencyID column of the TransferHistory table equals the Agency being reported
ToServiceSiteID column of the TransferHistory table equals the Clinic being reported

This value is a five-digit number.

Transfers Out

Origin of Data Element

This value is the count of unique State WICID's who transferred out of the clinic during the reported month. Participants must meet the following selection criteria to be included in this count:

EffectiveDate column of the TransferHistory table equals the reported month
FromAgencyID column of the TransferHistory table equals the Agency being reported
FromServiceSiteID column of the TransferHistory table equals the Clinic being reported

Filter Criteria

Base of Data

All clinic activity for the fiscal year will be included in the report. Data is reported for all months from the start of the fiscal year to the month selected. Data for hidden agencies will not be included.

The values for the Food Benefit Pickup, Education Contacts, Transfers In and Transfers Out are counts of unique State WICID's for both the clinic and state totals. Participants that receive these benefits from more than one clinic within the same month are counted in the value for both clinics. However, the participants are counted only once in the state totals. Because of this, the sum of the clinic values will not match the state totals.

The values for the other lines of this report are based on data from the CaseloadCount and CaseloadCountEnrollment tables. The month-end process updates these tables and ensures an unduplicated count for each month, regardless of the transfer of participants within the month.

Sort Order

Agency

The report is sorted first by Agency.

Clinic ID

The report will be sorted by the Clinic ID within Agency ID.

Control Breaks

Agency

A page break will occur on change in Agency.

Clinic

A page break will occur on change in clinic.

Grand Total

State totals will be printed at the end of the report.

Frequency and Distribution

This system output is generated on demand.
