The WIC Closeout Summary Report FIN021 (Output) reflects the final disposition (status) of checks so that the final bank reconciliation can be completed. The report also provides a summary by WIC category showing a count of checks redeemed, the value of the redeemed checks, and the average value per check.
The report is generated when the OK button is clicked on the Generate WIC Closeout Summary Report screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
Origin of Data Element
The user records this value when the report is submitted. During the month-end process this is the CCYYMM value from the CloseMonthEndDt attribute of the ProcessControl table.
Format
This is an accounting month and year formatted as Month Name CCYY.
Origin of Data Element
The selection criteria and information being reported determine this value.
When reporting paid without issuance (first page of report), the clinic, agency, and state total descriptions will be replaced by the text "Paid Without Issuance".
When reporting clinic data, the agency code, clinic code and clinic description will be printed.
When reporting agency data, the agency code and description will be printed.
When reporting state data, the text description of "State Total" will be printed.
Format
This is a fifty-character text field. The format is determined by the information being reported.
Origin of Data Element
This value is a count of food instruments selected for the report that have a not null value in the PaidDate attribute of the FoodInstrument table and a value of "P", "B", or "N" in the WICStatus attribute of the FoodInstrumentSet table.

This is a six-digit number with no decimal places.
Origin of Data Element
This value is a count of food instruments selected for the report that have a not null value in the PaidDate attribute of the FoodInstrument table and a value of "C" in the WICStatus attribute of the FoodInstrumentSet table.

This value comes from summing the PaidAmt attribute of the FoodInstrument table for checks selected for the report that have either an "I" or "F" in the WICStatus attribute of the FoodInstrumentSet table.

This value comes from summing the counts of number of checks redeemed, number of checks lot/stolen, number of checks voided, and total checks unredeemed.

Food instruments that meet the following selection criteria are included in the report.
Paid Without Issuance Data (no agency/clinic)
The MMCCYY portion of the PaidDate attribute of the FoodInstrument table is equal to the MMCCYY portion of the 'month of' selected by the user.
The IssuedDate attribute of the FoodInstrumentSet table is null.
The ProgramCode attribute of the FoodInstrument table is equal to the WIC value.
Participation Detail
The ParticipationDate attribute of the FoodInstrumentSet table is greater than zero and is during the, 'month of' selected by the user.
The ProgramCode attribute of the FoodInstrument table is equal to the WIC value.
A page break will occur on change in clinic/agency. Clinic totals will print on a change in clinic. Agency totals will print on a change in agency.
State totals will be printed.
This system output is generated on demand. If selected by the state, this system output is also generated during the month-end process.
