The VOC Document (Output) allows the user to print a summary report of participant information at a clinic for use as a Verification of Certification (VOC) document. The report is generated when the OK button is clicked on the Produce VOC Document screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
The title of the report will be the text "State Department of Health" and include your state name. The first subtitle will be the text "WIC Program". The second subtitle will be the text "Verification of Certification". The third subtitle will be the full name of the participant.
Origin of Data Element
The full name of the participant will be taken from the Last-Name, First-Name, and Middle-Initial columns of the MEMBER table.
Format
The value will print in {first name} {middle initial}. {last name} format.
This value will be the State WIC ID of the participant.
Origin of Data Element
The value will be taken from the State-WIC-ID column of the MEMBER table.
Format
The value will print as its literal value.
This value will be the Household ID of the participant.
Origin of Data Element
The value will be taken from the Household-ID column of the MEMBER table.
Format
The value will print as its literal value.
The value will be the date of birth declared by the participant as an element of enrollment.
Origin of Data Element
The value will be taken from the DateOfBirth column of the MEMBER table.
Format
The value will print as its literal value.
This value will be the date on which income eligibility for the current certification period was determined.

The value will be taken from the ScreenDate column of the INCOMECONTACT table.
Format
The value will print as its literal value.
This value will be the date from which the participant is certified and can utilize WIC benefits.
Origin of Data Element
The value will be taken from the CertEffectiveDate column of the CERTCONTACT table.
Format
The value will print in MM/DD/CCYY format.
This value will be the end date after which the participant is no longer certified and cannot utilize WIC benefits.

The value will be taken from the CertEndDate column of the CERTCONTACT table.
Format
The value will print in MM/DD/CCYY format.
Origin of Data Element
This value comes from the Description column of the WICStatus table associated with the WICStatus column of the Member table.
Format
The value will print as its literal value. This is a 20-character value.
The value will be the priority determined for the WIC participant for the current certification period based on determined risk factors.
Origin of Data Element
The value will be derived from the Cert-Assigned-Priority column of the CERTCONTACT table.
Format
The value will print as its literal value.
The value will be the date that blood work was completed for the participant.
The system uses the last height/weight and blood work records recorded for the participant to populate the height/weight and blood work information on the generated Verification of Certification (VOC) document. The data for the Verification of Certification (VOC) document is retrieved from the AnthropContact and BloodWork tables.
Origin of Data Element
The value will be taken from the ResultDate column of the BLOODWORK table.
Format
The value will print in MM/DD/CCYY format.
The value will be the hemoglobin value of the blood work results of the participant.

The value will be taken from the Hemoglobin column of the BLOODWORK table.
Format
The value will print in ##.# format.
The value will be the hematocrit value of the blood work results of the participant.

The value will be taken from the Hematocrit column of the BLOODWORK table.
Format
The value will print in ##.# format.
The value will be the lead value of the blood work results of the participant.
Origin of Data Element
The value will be taken from the Lead column of the BLOODWORK table.
Format
The value will print in ## format.
The value will be the E.P. value of the blood work results of the participant.
Origin of Data Element
The value will be taken from the EP column of the BLOODWORK table.
Format
The value will print in ## format.
The value will be the reason why blood work measurements were not collected for the participant

Format
The value will print as its literal value.
The value will be the date that the height/weight measurements were taken.
The system uses the last height/weight and blood work records recorded for the participant to populate the height/weight and blood work information on the generated Verification of Certification (VOC) document. The data for the Verification of Certification (VOC) document is retrieved from the AnthropContact and BloodWork tables.
Origin of Data Element
The value will be taken from the MeasurementDate column of the ANTHROPCONTACT table.
Format
The value will print in MM/DD/CCYY format.
The value will be the height of the participant in standard terms of feet and inches. Inches may include eighths of inches as in the case of infants.
Origin of Data Element
The value will be taken from the LengthInches and LengthEighths columns of the ANTHROPCONTACT table. The value of the LengthInches column will be divided by twelve to derive the number of feet. The remainder of inches and eighths will be stated together in one component (i.e. 6 2/8). If there are zero (0) eighths of an inch to report, the eighths measurement is not included.
Format
The values will print in a concatenated format of measurement and literals. For example, "1 foot 2 3/8 inches" would be derived from 14 inches and three eighths. Or, "1 foot 2 inches" would be derived from 14 inches and zero eighths.
The value will be the number of pounds and ounces of the weight of the participant.
Origin of Data Element
The value will be taken from the WeightPounds and WeightOunces columns of the ANTHROPCONTACT table.
Format
The values will print in a concatenated format of measurement and literals. For example, "147 pounds and 3 ounces" would be derived from 147 and 3 respectively in the table fields of WeightPounds and WeightOunces.
The value will be the risk factors assigned to the participant for the current certification. The risk factors may be system applied or manually applied.
Origin of Data Element
The value will be the lookup value from the Reference Dictionary table for the RiskFactorID column of the RISKFACTOR table.
Format
The value will print as its literal value.
This value label will alert the user that the household participant is considered to be high risk.
Origin of Data Element
The value label will contain the text "(High Risk)" if the risk factor is considered to be high risk.
Format
The value will print as its literal value.
This value will be the first date that the food instrument can be used by the participant.
Origin of Data Element
The value will be taken from the First-Use-Date column of the FOOD-INSTRUMENT-SET table.
Format
The value will print in MM/DD/CCYY format.
This value will be the last date that the food instrument can be used by the participant.
Origin of Data Element
The value will be taken from the Last-Use-Date column of the FOOD-INSTRUMENT-SET table.
Format
The value will print in MM/DD/CCYY format.
This value will be an aggregate count of the number of instruments received by the participant at that print date.
Origin of Data Element
The value will be a summary of the number of food instruments issued for each set of checks displayed.
Format
The value will print in ### format.
This value will be the local agency where the participant was enrolled.
Origin of Data Element
The value will be taken from the look up database.
Format
The value will print as its literal value.
This value will be the address of the local agency for which the participant was enrolled. It will consist of the street address, city, state, and zip code.
Origin of Data Element
The value will be taken. The Value of your state "AA" will be used as the state.
Format
The value will print as its literal, concatenated value.
This value will be the telephone number of the Clinic the participant may call.
Origin of Data Element
The value will be taken from the look up database.
Format
The value will print as its value in format ###-###-####.
Only information for the selected participant will be included in the report.
Only blood work results, anthropometric measurements, risk factors, and food instruments recorded for the participant during the current certification period will be included in the report.
Only food instruments that were issued, and not voided or reported stolen, will be included in the report.
The food instruments will be sorted by the first date that the food instrument can be used.
