The Upcoming Appointments Report CLN020 (Output) allows the user to print a report of the upcoming appointments during a specified date range. The report is generated when the OK button is clicked on the Generate Upcoming Appointments Report screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
Origin of Data Element
The user records the from and to dates when the report is submitted.
Format
The dates will print in MM/DD/CCYY format.
Origin of Data Element
The code value is the AgencyID column of the Member table for the selected records. The associated name value is the Name column of the Agency table for the corresponding code
Format
The code is a three character value and the name is a fifty character value.
Origin of Data Element
The code value is the ServiceSiteID column of the Member table for the selected records. The associated name value is the ServiceSiteName column of the ServiceSite table for the corresponding code.

The code is a three-character value and the name is a fifty-character value.
Origin of Data Element
This value is the resource and will be taken from the Name column of the AppointmentResource table for the AppointmentResourceID associated with the appointment.
Format
This is a fifty-character value.
Origin of Data Element
This value is the date of the appointment and will be taken from the AppointmentDate column of the Appointment.
Format
The value will print in MM/DD/CCYY format.
Origin of Data Element
The value will be taken from the AppointmentTime column of the Appointment table.
Format
The value will print in HH:MM {AM/PM} format.
Origin of Data Element
The value will be taken from the StateWICID column of the Member table.
Format
This is an eight-character value.

The value will be taken from the HouseholdID column of the Member table.
Format
This is an eight-character value.

The value will be taken from the FirstName, MiddleInitial and LastName columns of the Member table.
Format
The value will print in {first name} {middle initial}. {last name} format.
Origin of Data Element
The value will be taken from the Telephone1 column of the Household table.
Format
This is a ten-character value formatted XXX-XXX-XXXX.
Origin of Data Element
The value will be taken from the InterpreterNeeded column of the Household table.
Format
This is a one-character value.
Origin of Data Element
The value will be taken from the Disability column of the Member table. If the value is null, 'N/A' will be printed.
Format
This is a one-character value.
Origin of Data Element
This value is the appointment type and will be taken from the Description column of the AppointmentType table for the AppointmentTypeID associated with the appointment.
Format
This is a fifty-character value.
Participants with appointments scheduled at the specified clinic(s) between the starting and ending dates, inclusive, are included in the report.
If the Group Report by Resource check box on the Generate Upcoming Appointments screen was selected, the report will be sorted by resource name within clinic ID.
The report will be sorted by appointment date.
If the Group Report by Resource check box on the Generate Upcoming Appointments Report screen was selected, the sort by appointment date will be within resource name.
If the Group Report by Resource check box on the Generate Upcoming Appointments Report screen was not selected, the sort by appointment date will be within clinic name.
The report will be sorted by appointment time within appointment date.
On a change of clinic, a page break will occur.
If the Group Report by Resource check box on the Generate Upcoming Appointments Report screen was selected, a page break will occur on a change of a resource name.
On a change of appointment date, a page break will occur and a count of appointments will print.
A summary page with the grand total count of appointments included in the report will print.
