Peer Counselor Contact Pending - Breastfeeding Report CLN008 (Output)

The Peer Counselor Contact Pending - Breastfeeding Report CLN008 (Output) includes all Breastfeeding participants who have not been contacted by a breastfeeding peer counselor and their actual delivery date falls within the given date range. The report is generated when the OK button is clicked on the Generate Peer Counselor Contact Pending - Breastfeeding Report screen.

Origin of Data Element

This value is the StateWICID column of PostPartum table for selected participants.

Format

This is an eight-character value.

HH ID (Household ID)

This value is the HouseholdID column of Member table for selected participants.

Format

This is an eight-character value.

Participant Name/Infant Name

Origin of Data Element

The participant name is the LastName, FirstName and MiddleInitial columns of Member table for the selected participants. The infant name is the LastName, FirstName and MiddleInitial columns of Member table for child of selected postpartum record. If the pregnancy resulted in multiple births, only one child is listed at a time. The other children are listed with their own set of information.

Format

These values are 25-character, 20-character and one-character values, respectively. The text is formatted as Last, First MI.

Address/Telephone

Origin of Data Element

This value is the MailAddress, MailCity, MailState, MailZip and Telephone1 columns of Household table for the selected participants.

Format

The MailAddress is a 50-character value. The MailCity is a 30-character value. The MailState is a two-character value. The MailZip is a nine-character value formatted XXXXX-XXXX. The Telephone1 Is a ten-character value formatted XXX-XXX-XXXX.

Actual Delivery Date

Origin of Data Element

This value is the ActDelivDate column of Postpartum table for selected participants.

Format

This is a date value formatted MM/DD/CCYY.

Breastfeeding Duration

Origin of Data Element

This value is the difference in days between the ActDelivDate column of Postpartum table and the current date.

Format

This is a three character value formatted "XXX days".

Amount of Formula Issued

Origin of Data Element

This is the total quantity in ounces of formula issued to the child for the last full month of issuance. Late pickups that cause partial packages are not accounted for.

• Food instruments are selected that meet the following selection criteria.

• ParticipationDate of the FoodInstrumentSet equals the CCYYMM portion of the CurrentMonthEndDt column of the ProcessControl table.

• The base food category ID associated with the FoodItemID column of the FoodInstrumentItem table equals "A" (formula).

• The quantity of the food instrument item is multiplied by the ContainerFormulationUnits of the asociated FoodItem table. The result is totaled and rounded to the nearest whole number.

Format

This is a five-digit value formatted as "XX,XXX oz".

Age Formula Introduced

Origin of Data Element

This value the difference in months/days between the ActDelivDate column of Postpartum table and the SuppFeedBeganDate column of the PostPartumInfant table for the selected child. If formula has not been introduced, the value is blank.

This is a text value that prints as a literal value.

Last Date to Use Last Set of Benefits

Origin of Data Element

This value is the LastCheckLDTU column of Member table for selected participants.

Format

This is a date value formatted MM/DD/CCYY.

Date of Last Peer Counselor Contact

Origin of Data Element

This value is the most current ContactDate from the BFCounselorContact table for the breastfeeding woman (most current contact date less than or equal to the current date). This may be a contact that occurred during the pregnancy.

This is a date value formatted MM/DD/CCYY.

Peer Counselor

Origin of Data Element

This value is the LastName, FirstName and MiddleInitial columns of the BFCounselor table for the counselor associated with the last peer counselor contact date.

Format

These values are 25-character, 20-character and one-character values, respectively. The text is formatted as Last, First MI.

Date of Last Referral

Origin of Data Element

This value is the most current ReferralDate from the PeerCounselorReferralItem table for the breastfeeding woman (most current referral date less than or equal to the current date).

Format

This is a date value formatted MM/DD/CCYY.

Filter Criteria

Base of Data

Breastfeeding women that meet the following selection criteria are included in the report:

All Participants

• WICStatus column of the Member table must equal "B".

• The ActDelivDate column of the Postpartum table is with the selected From Date and To Date, inclusive.

• Participant is in a valid certification at the time the report is requested.

Participants with No Contacts

• WICStatus column of the Member table must equal "B".

• The ActDelivDate column of the Postpartum table is with the selected From Date and To Date, inclusive.

• Participant is in a valid certification at the time the report is requested.

• The participant does not have a record in the BFCounselorContact table where the ContactDate column is greater than or equal to the ActDelivDate of the selected PostPartum table.  (The participant has not had a contact since her delivery date.)

Clinic, agency, grantee, state

• If clinic report, AgencyID and ServiceSiteID columns of the Member table equal selected agency/clinic ID(s).

• If agency report, AgencyID column of the Member table equals the selected agency ID.

• If grantee report, AgencyID column of the Member table equals on of the AgencyID column of the GranteeClinic tablefor the selected grantee.

• If state report, hidden agencies are not included.

Sort Order

Clinic / Agency / Grantee / State

• If report by clinic and one clinic is selected, this part of the sort order does not apply.

• If report by clinic and multiple clinics are selected, the report will be sorted by clinic ID.

• If report by agency is selected, this part of the sort order does not apply.

• If report by grantee is selected, this part of the sort order does not apply.

• If state is selected, this part of the sort order does not apply.

Actual Delivery Date

Within Clinic / Agency / Grantee / State, the report will be sorted by Actual Delivery Date.

State WIC ID

Within Actual Delivery Date, the report will be sorted by State WIC ID of breastfeeding woman.

Infant Name

Within State WIC ID of breastfeeding woman, the report will be sorted by Infant Name.

Control Breaks

Clinic / Agency / Grantee / State

A page break will occur on the change of clinic with one of the following totals depending on selection criteria.

• Total breastfeeding participants at clinic not contacted since delivery.

• Total breastfeeding participants at clinic.

Grand Total

A report total will print at the end of the report. If multiple clinics are selected, this is the total for all selected clinics. If one clinic, agency, grantee or state totals are selected, this is the total for the one clinic, agency, grantee or state.
