The Participant High Risk Report CLN030 (Output) consists of data for individual clinics, agency summaries and the state summary. The report is generated when the OK button is clicked on the Generate Participant High Risk Report screen.
The clinic report includes participant information for high risk participants in a valid certification during the reported month. Participants are grouped as those with a future appointment and those without a future appointment. The appointment does not have to be at the agency/clinic that the participant is currently assigned to. The agency reports is a one-line summary for each clinic within the agency listing counts of high risk participants in a valid certification during the reported month with and without future appointments plus a line for the agency totals. The State report includes an agency report for each agency plus a one-line total for the state.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
Origin of Data Element
The user records this value when the report is submitted.
Format
This is a calendar month and year formatted as Month Name CCYY.
Origin of Data Element
The selection criteria and information being reported determine this value.
When reporting clinic data, the agency code, agency description, clinic code and clinic description will be printed.
When reporting agency data, the agency code and description will be printed.
When reporting state data, the text description of "State Total" will be printed.
Format
This is a fifty-character text value. The format is determined by the information being reported.
Origin of Data Element
This value comes from the StateWICId column of the CertContact table for the selected participants.
Format
This value is an eight-character value.
Origin of Data Element
The name value comes from the LastName, FirstName and MiddleInitial columns of the Member table for the selected participants.
Format
The value will appear in last name, first name, and middle initial format.
Origin of Data Element
This value comes from the MailAddress, MailCity, MailState and MailZIP columns of the Household table for the selected participants.
Format
This is a character value formatted address, city, st zip.
Origin of Data Element
This value comes from the Telephone1 column of the Household table for the selected participants.
Format
This is a ten-character value formatted XXX-XXX-XXXX.

This value comes from the WICStatus column of the Member table for the selected participants.
Format
This is a one-characer value.

This value comes from the DateOfBirth column of the Member table for the selected participants.
Format
This is a date in the format of MM/DD/CCYY.
Origin of Data Element
This is the appointment type, date and time of the next appointment for the participant. The values come from the AppointmentTypeId, AppointmentDate and AppointmentTime columns of the Appointment table.
Format
The appointment type is a fifty-character value. The appointment date is in the format of MM/DD/CCYY. The appointment time is in the format of MM:SS with an AM/PM suffix.
Origin of Data Element
This is the appointment type, date and time of the last appointment for the participant. The values come from the AppointmentTypeId, AppointmentDate and AppointmentTime columns of the Appointment table. If the participant has not had any appointments, the type, date and time will be blank on the report.

The appointment type is a fifty-character value. The appointment date is in the format of MM/DD/CCYY. The appointment time is in the format of MM:SS with an AM/PM suffix.

These values come from the CertEffectiveDate and CertEndDate columns of the CertContact table for the selected participants.
Format
Both are dates in the format of MM/DD/CCYY.
Origin of Data Element
This is the list of risk factors assigned to the participant for this certification. The value comes from the Description column of the RiskFactorReference table.

This is a sixty-characer value.

This is the count of selected participants with future appointments.
Format
This is a six-digit number.

All high risk WIC participants that are in a valid certification during the selected month are selected. Participants that meet the following critieria are selected:
The CertStartDate column of the CertContact table is less than or equal to the last day of the selected month.
The CertEndDate column of the CertContact table is greater than or equal to the first date of the selected month.
The CertTermDate column of the CertContact table is either null or greater than or equal to the first date of the selected month.
The HighRisk column of the CertContact table is "Y".
Participants are counted once based on their most current certification. If a participant's certification ends and restarts during the selected month, the certification with the highest certification effective date will be used. If a participant transfers between agencies or clinics, the certification with the highest certification effective date will be used.
Participants are grouped based on their agency/clinic values on the member table, not where they were certified. Participants are grouped based on whether or not they have a future individual or group appointment. Participants with an appointment greater than or equal to the current date are considered to have future appointments. The time of this appointment is not used to determine future appointments. The appointment does not have to be at the agency/clinic that the participant is currently assigned to.
Data for hidden agencies will not be included.
For the clinic report: if more than one clinic is selected, the report will be sorted by clinic ID. The participants with appointments will print in a group before the participants without appointments. Participants with appointments will be sorted by appointment date and time in descending sequence, then state WICID. Participants without appointments will be sorted by appointment date and time in descending sequence, then state WICID. The risk codes listed under Reason for High Risk will be sorted by the Description column of the RiskFactorReference table.
For the agency report: the agency total will print first. The totals for clinics within the agency will print in clinic number sequence.
For the state report: the agency reports will print for all agencies, sorted by agency ID.
