The Certification Due Address Labels (Output) can be generated for all clients who will receive a certification due notice for mailing purposes and are generated when the Address Labels section is completed and the OK button is clicked on the Generate Certification Due Report/Letters screen.
This section describes the data elements that print on the system output. Because of space constraints, the layout may be slightly different and/or all characters may not print within the space allotted.
Origin of Data Element
The value will be taken from the Member.StateWICID.
Format
The value will print as its literal value.
Origin of Data Element
The value will be taken from the Member.FirstName Member.MiddleInitial and Member.LastName.
Format
The value will print in {first name} {middle initial}. {last name} format.
Origin of Data Element
The value will be taken from the Mail-Address of the HOUSEHOLD table of the client.
Format
The value will print as its literal value.
Origin of Data Element
The value will be taken from the Mail-City of the HOUSEHOLD table of the household member.
Format
The value will print as its literal value.

The value will be taken from the Household.MailState.
Format
The value will print as its literal value.
Origin of Data Element
The value will be taken from the Household.MailZIP.
Format
The value will print in #####-#### format.
The system will generate an address label for each client for which a subsequent certification appointment notice was generated.
Only one address label will be produced per household when subsequent certification appointment notices are generated for multiple members of the same household.
The address labels will be generated in the last name, first name, middle initial order.
