The Medical Prescription - Formula Report CAS012/CLN034/OPR011 (Output) is a state level report that shows the total number of participants that have been issued special formula food items. All participants that are in a valid certification period and were issued special formula during the reported month are included in the report. If a participant is issued multiple types of special formula, the participant is counted in the total for each type of special formula. A participant can be counted more than once.

The report is generated when the OK button is clicked on the Generate Medical Prescription - Formula Report screen. This report will also be generated as part of the month-end process.

This value is the sum of infants that were prescribed the prescription item in a clinic for the selected month.

This value is the percentage of infants that were prescribed the prescription item in a clinic for the selected month.

This is a three-digit percentage with two decimal places.

Women & Child Count

Origin of Data Element

This value is the sum of women and children that were prescribed the prescription item in a clinic for the selected month.

This is a six-digit number.

Women & Children %

Total Count

This value is the sum of infants, women, and children that were prescribed the prescription item in a clinic for the selected month.

Sort Order

Grantee ID / Agency ID / Clinic ID / Prescription Item

This report is sorted first by the Grantee ID, then the Agency ID, Clinic ID, and finally the Prescription Item.

Control Breaks

Grantee ID

A page break will occur on change in Grantee.

Agency ID

A page break will occur on change in Agency.

Clinic ID

A page break will occur on change in Clinic.

Prescription Item

A page break will occur on change in Prescription Item.

Grand Total

Infant Count

This is the sum of all infants in the report that were prescribed the prescription item.

Infant %

This is the total percentage of all infants in the report that were prescribed the prescription item.

Women & Children Count

This is the sum of all women and children in the report that were prescribed the prescription item.

Women & Children %

This is the total percentage of all women and children in the report that were prescribed the prescription item.

Total Count

This is the sum of all infants, women, and children in the report that were prescribed the prescription item.

Frequency and Distribution

This system output is generated on demand.
