Direct Shipment Formula Receipt Report (Output)

The Direct Shipment Formula Receipt Report (Output) lists the direct formula item as a receipt that can be given to the participant. This report uses a customizable template that includes merged fields. It is generated when the Print Receipt button is clicked on the Benefits History screen.

Data Elements

StateWICID

This value comes from the StateWICID column of the FoodInstrumentSet table.

This value prints as the literal value.

ParticipantName

Origin of Data Element

This value comes from the FirstName, LastName, and MiddleInitial columns of the Member table.

Format

This value prints as the literal value.

ParticipantPhoneNumber

Origin of Data Element

This value comes from the Telephone1 column of the Household table.

Format

This value prints in ###-###-#### format.

ShipToAddress

Origin of Data Element

This value comes from the ShipToAddress column of the FoodInstrument table.

This value comes from the ShipToCity column of the FoodInstrument table.
