The Child Health Information screen is used to access and maintain the current infant or child client's health information.

The Child Health Information screen displays when the WIC Category of the participant is an Infant or a Child.

Screen Access

  • Display the Participant Folder screen for an infant or a child client > Click the Health Information tab.

Interface Initialization

Upon initial display of the screen, the following occurs:

  • The title bar text is set to "MEMBER.FIRSTNAME MEMBER.MiddleInitial MEMBER.LASTNAME - {descriptive value of age} - WIC ID: MEMBER.STATEWICID Household ID: MEMBER.HOUSEHOLDID", where {descriptive value of age}equals:

  • For infants, display "XX" Months "XX" Days, calculated from the Date of Birth.

  • For children, display "XX" Year(s) "XX" Months "XX" Days calculated from the Date of Birth.

  • For pregnant women, display "XX" weeks gestation calculated from the LMP start date.

  • For pregnant women without an LMP date, display "XX" Year(s) calculated from the Date of Birth.

  • For breastfeeding and non-breastfeeding women, display "XX" Years calculated from the Date of Birth.

  • The tab title is set to "Health Information".

  • All data controls are disabled and display the previously saved values.

  • If the value of the CLN_ALLOW_UNKNOWN_BIRTH_HT_WT business rule is set to "Y", the Unknown Birth Criteria check box is visible. If the value of the CLN_ALLOW_UNKNOWN_BIRTH_HT_WT business rule is set to "N", the Unknown Birth Criteria check box is not visible.

  • The current system date and time is displayed in the status bar at the bottom right of the screen.

Birth Information group box

  View the information in the Birth Information group box.

This group box includes controls that allow you to view birth information for the infant or child. The title text of the group box is set to "Birth Information". The following controls are included in the group box:

Unknown Birth Criteria check box

This control indicates whether the client's birth information is unknown as recorded during certification. Information in this control is read-only and cannot be changed.

NOTE: Depending on your WIC Program, the Unknown Birth Criteria check box may not be available.

Birth Height In masked edit box

This control displays the client's height at birth in whole inches as recorded during certification. Information in this control is read-only and cannot be changed.

Birth Height 8ths masked edit box

This control displays the remainder of the client's height at birth in eighths of an inch as recorded during certification. Information in this control is read-only and cannot be changed.

Birth Weight Lbs masked edit box

This control displays the client's weight at birth in whole pounds as recorded during certification. Information in this control is read-only and cannot be changed.

Birth Weight Ozs masked edit box

This control displays the remainder of the client's weight at birth in ounces as recorded during certification. Information in this control is read-only and cannot be changed.

Preterm or Early Term Delivery check box

This control indicates whether the client was born prematurely as recorded during certification. Information in this control is read-only and cannot be changed.

Gestation group box

This group box includes controls that allow you to view gestation information for the infant or child. The title text of the group box is set to "Gestation". The following controls are included in the group box:

Weeks masked edit box

View the number of weeks the client spent within the womb.

Weeks text and value label

The text and value label displays the total weeks of gestation for the infant or child.

The control is visible and enabled when:

The information displayed is read-only. The value displays in the inverse color of the screen.

Days text and value label

The text and value label displays the total days of gestation for the infant or child.

The control is visible and enabled when:

The information displayed is read-only. The value displays in the inverse color of the screen.

Birth Facility drop-down list box

This control displays the name of the medical facility at which the client was born as recorded during certification. Information in this control is read-only and cannot be changed.

Mother's Information group box

  View the information in the Mother's Information group box.

This group box includes controls that allow you to view the mother's information for the infant or child. The title text of the group box is set to "Mother's Information". The following controls are included in the group box:

Date of Birth calendar control

This control displays the date on which the client's mother was born as recorded during certification. Information in this control is read-only and cannot be changed.

On WIC check box

This control indicates whether the client's mother participates in the local WIC program as recorded during certification. Information in this control is read-only and cannot be changed.

State WIC Information group box

This group box includes controls that allow you to view details of the mother linked to the infant or child. The title text of the group box is set to "State WIC Information". The following controls are included in the group box:

ID text and value label

If the child is linked to a mother who participates in the WIC program, the text and value label displays the WIC identification number of the child's mother. It is filled with the value of the MOTHERSTATEWICID column of the CHILD table. If the child is not linked, it is filled with the value of N/A.

The information displayed is read-only. The value displays in the inverse color of the screen.

Name text and value label

If the child is linked to a mother who participates in the WIC program, the text and value label displays the name of the child's mother. It is filled with the value of the FIRSTNAME, MIDDLEINITIAL, and LASTNAME columns of the MEMBER table. If the child is not linked, it is filled with the value of N/A.

The information displayed is read-only. The value displays in the inverse color of the screen.

Feeding Information group box

  View the information in the Feeding Information group box.

This group box includes controls that allow you to view the feeding information for the infant or child. The title text of the group box is set to "Feeding Information". The following controls are included in the group box:

Ever Breastfed radio button group

This radio button group includes controls that allow you to view whether the applicant has ever been breastfed as recorded during certification. The title text of the radio button group is set to "Ever Breastfed". The following radio buttons are included in the group:

Yes radio button

This control indicates if the client has been breastfed as recorded during certification.

No radio button

This control indicates if the client has never been breastfed as recorded during certification.

Unknown radio button

This control indicates it is not known whether the client has ever been breastfed as recorded during certification.

Requires Food Package III check box

This control indicates whether the client requires a Food Prescription III assignment as recorded during certification. Information in this control is read-only and cannot be changed.

Date Food Package III Verified calendar control

This control displays the date on which the client's Food Prescription III assignment verification occurred as recorded during certification. Information in this control is read-only and cannot be changed.

Breastfeeding Now check box

This control indicates whether the client is currently breastfeeding as recorded during certification. Information in this control is read-only and cannot be changed.

Breastfeeding beyond One Year check box

This control indicates whether the client is currently breastfeeding and is one year of age or older, as recorded during certification. Information in this control is read-only and cannot be changed.

Date Breastfeeding Verified calendar control

This control displays the date on which the client's breastfeeding status was verified as recorded during certification. Information in this control is read-only and cannot be changed.

Breastfeeding Amount drop-down list box

This control displays the amount the client is breastfed as recorded during certification. Information in this control is read-only and cannot be changed.

Date Breastfeeding Began calendar control

This control displays the amount the date on which the client began breastfeeding as recorded during certification. Information in this control is read-only and cannot be changed.

Date Breastfeeding Ended calendar control

This control displays the date on which the client ceased to breastfeed as recorded during certification. Information in this control is read-only and cannot be changed.

Reason(s) Stopped list box

This control displays the reasons the client ceased to breastfeed as recorded during certification. Information in this control is read-only and cannot be changed.

Date Supplemental Feeding Began calendar control

This control displays the date on which anything other than breast milk was introduced into the infant's diet as recorded during certification. Information in this control is read-only and cannot be changed.

Date Solids Were Introduced calendar control

This control displays the date on which solid foods were introduced into the infant's diet as recorded during certification. Information in this control is read-only and cannot be changed.

  View the information in the Health Information group box.

This group box includes controls that allow you to view the health information for the infant or child. The title text of the group box is set to "Health Information". The following controls are included in the group box:

Household Smoking drop-down list box

This control displays the presence of environmental cigarette smoke in the client's household. Information in this control is read-only and cannot be changed.

TV/Video Viewing drop-down list box

This control displays the amount of television and video viewing that occurs within the client's household. Information in this control is read-only and cannot be changed.

Medical Conditions group box

  View the information in the Medical Conditions group box.

This group box includes controls that allow you to view medical conditions for the infant or child. The title text of the group box is set to "Medical Conditions". The following controls are included in the group box:

Diabetes Mellitus check box

This check box indicates the presence of diabetes mellitus as recorded during certification. Information in this control is read-only and cannot be changed.

Hypertension or Prehypertension check box

This check box indicates the presence of hypertension or prehypertension as recorded during certification. Information in this control is read-only and cannot be changed.
